| Medical Forms | |
| Class 1 forms are required annually by all participants, both scouts and adult leaders. This form must be current to participate in camping or other activities not exceeding 72 hours. | Class 1 Only PDF(2.9KB) Updated 05/13/07 |
| Class 2 forms are required once every 36 months for all participants under
40 years of age. This form must be current to participate in
activities exceeding 72 hours. Exceptions: Some Scouting activities, such as Summer camp, require this form to be current and on file even if the parent or adult leader is only staying one night. |
